Website Privacy Policy

This Privacy Policy describes how Gulfside Living Healthcare Services, LLC ("we," "us," or "our") collects, uses, and shares personal information when you visit our website at www.gulfsidelivinghealthcare.com (the "Site"). It does not cover Protected Health Information (PHI) governed by our HIPAA Notice of Privacy Practices, which is posted separately.

Information We Collect

We collect three categories of information through the Site:

  • (a) Information you provide. When you submit a Care Request, Service Request, or Contractor Inquiry form, we collect the information you provide, such as name, email address, phone number, address, discipline, license information, and the details of your request or inquiry. We may also collect optional information you choose to share.
  • (b) Information collected automatically. When you visit the Site, we automatically collect limited technical information, including IP address, device type, browser type, pages visited, referring URL, and the date and time of your visit. We use cookies and similar technologies for basic site analytics and performance.
  • (c) Information from third parties. If you contact us through a third-party platform (for example, by clicking a link from Google Business Profile or a social media account), we may receive limited information from that platform.

How We Use the Information

  • To respond to your inquiry, request, or application
  • To provide and improve the Site and our services
  • To send transactional communications (such as confirmations and follow-ups related to your inquiry)
  • To comply with legal obligations, including AHCA and HIPAA requirements
  • To detect, prevent, and address fraud, abuse, or security incidents

How We Share the Information

We share information only as needed to operate our services and as described in this Policy. Specifically, we may share information with:

  • Service providers and business associates who help us operate the Site (web hosting, email, analytics) under contracts that protect your information
  • Independent contractors when you have requested a referral and information must be shared to coordinate care
  • Specialty pharmacies, physician practices, or facilities that requested coverage from us, when needed to fulfill the request
  • Government agencies, regulators, or law enforcement when required by law or in response to lawful process

We do not sell or rent personal information to advertisers or marketers. We do not share PHI for marketing purposes without your written authorization.

Cookies and Analytics

Our Site uses essential cookies for security and basic functionality, and optional analytics cookies (Google Analytics or similar) to understand how visitors use the Site. You can manage cookies through your browser settings. Most browsers also allow you to enable a Do Not Track signal; we honor Do Not Track signals where technically feasible.

Data Security

We use reasonable administrative, technical, and physical safeguards to protect personal information. The Site uses HTTPS encryption for all form submissions. No method of transmission over the Internet is 100% secure, however, and we cannot guarantee absolute security.

Data Retention

We retain information for as long as necessary to fulfill the purposes for which it was collected, comply with legal obligations, resolve disputes, and enforce our agreements. For most website inquiries, this is 24 months unless you become a client or contractor, in which case retention is governed by Florida record-keeping requirements (typically five years after the last service date).

Children's Privacy

The Site is not directed to children under 13. We do not knowingly collect personal information from children under 13.

Your Rights

Depending on your state of residence, you may have rights to access, correct, delete, or restrict our use of your personal information. To exercise any of these rights, contact us at the address below. Florida residents may also exercise the consumer protections available under the Florida Digital Bill of Rights as those provisions take effect.

Changes to This Policy

We may update this Privacy Policy from time to time. The "Last Updated" date at the top of the Policy indicates when it was last revised. Material changes will be highlighted on the Site for a reasonable period.
