About Gulfside Living Healthcare Services

Our Mission — Bringing Healing Home

Gulfside Living Healthcare Services, LLC is a Florida nurse registry serving Charlotte, Lee, Collier, and Sarasota counties (AHCA Region 8). We refer credentialed, independently licensed nurses, certified nursing assistants, home health aides, and homemaker companions to private-pay clients and to contracting specialty pharmacies and physician practices. Our goal is simple: connect people who need skilled or supportive care in their own home with experienced, vetted Florida professionals who can provide it.

Gulfside Living Healthcare Services, LLC was founded in 2026 by two Lee Health-trained Registered Nurses with combined decades of acute-care, emergency, and primary-care experience. We started the Registry because we saw a gap in our community: families and contracting partners wanted reliable, named, fully credentialed nurses and caregivers — not just whoever was available — and clinicians wanted the flexibility of 1099 work without sacrificing professionalism. We built the Registry to serve both sides.

Our Founders

Felicia Hernandez-Fortunato, BSN, RN — Administrator, Financial Officer, Director of Nursing

Felicia is a Florida-licensed Registered Nurse and the Administrator, Financial Officer, and Director of Nursing for the Registry. She holds a Bachelor of Science in Management from Hodges University, an Associate of Science in Nursing from Florida Southwestern State College, and certifications in BLS, ACLS, PALS, TNCC, ENPC, and Charge Nurse Leadership. Her clinical background includes more than four years of Critical Care nursing at Lee Memorial Hospital, including three years as Charge RN (2021–2024), and current work as a Nurse Care Manager with Centene serving Medicare and Medicaid populations. Before nursing, Felicia served twenty-five years with Estero Fire Rescue as a Lieutenant Paramedic and Firefighter, retiring after a tour as Ride-Up Battalion Chief, and taught paramedic students at Medical Career Institute and Edison State College. Felicia leads the Registry's day-to-day operations, contractor credentialing, and care coordination.

Brandon L. Hawk, MSN, RN — Alternate Administrator

Brandon is a Florida-licensed Registered Nurse and Family Nurse Practitioner who serves as the Registry's Alternate Administrator. He holds a Master of Science in Nursing (Family Nurse Practitioner track) from Chamberlain University, a Bachelor of Science in Nursing and Associate of Science in Nursing from Mercy College of Northwest Ohio, and is FNP-certified by the American Academy of Nurse Practitioners. His clinical background includes Emergency Department nursing at Lee Memorial Hospital (since November 2022, including the Charge Nurse role from January 2024 to present), Specialty Transplant nursing at the Cleveland Clinic, and Neuro-Intermediate care at Mercy Health-St. Elizabeth, plus rotations in pediatric and primary-care practice. Brandon backs up the Administrator across all operating hours and is the on-call escalation contact for client and contractor emergencies.

How We Operate

We are a referral-based nurse registry: we do not employ caregivers as W-2 staff and we do not directly provide clinical care. All caregivers we refer are 1099 independent contractors who hold their own active Florida licenses or certifications, maintain their own continuing education, and carry their own professional liability insurance. We perform every contractor credentialing step required by Florida law — Level 2 background screening, OIG/SAM exclusion checks, license verification, references, skills competency, and BLS/ACLS/PALS where applicable — and we re-verify every twelve months. Our minimum liability-insurance requirement for clinical contractors is $1,000,000 per claim and $3,000,000 aggregate.

Gulfside Living Healthcare Services, LLC operates under a license pending issuance by the Florida Agency for Health Care Administration (File No. 42965622). Once issued, our license number will be displayed in the footer of this site.
